Subject: Placement of People with Special Needs
Question: I'm trying to develop a conceptual schema to help parents find placement for 
special needs family members. 

The possibilities are Intermediate Care Facilities (ICF), Group homes, Supported Apartments 
and Day Treatment/Living at Home. 

The entities involved are caregiver, staff, special needs person. 

These facilites are paid for by Social Security. 

The possible outcomes are accepted, rejected and waitlist. 

The special needs person is ineligble for public free services after age 22. 

The transitioning period is form age 18-22. 

I'm having difficulty constructing a model for this concept that is so sorely needed for 
people in this circumstance. 

I'm not sure how the data model should look. 

Can you please help?
