Data Model for Young People and Social Services
An Access Database is available on demand.
The Specifications have been provided in a question to Database Answers.

If you are new to Data Models, this page of my new Tutorial will help you understand the Data Model.
There might also be a design for a Data Mart, which is very useful for providng data for Reports and Business Intelligence.

Young People and Social Services Data Model
In this model we are concerned with Young People who may be in difficult family situations
where Schools or Churches might be involved in monitoring or helping.

Then these Young People might become what are called (in England) Youth Offenders, when
Social Workers or Case Workers might get involved, to make Assessments and Interventions.

This Data Model incorporates features from these three that I created a few years ago :-
  • Family Advocacy (2009) This includes Families, Goals, Individuals and Progress in School.
  • Offenders and Movements (2009) This includes Associates, Events, Offenders and Staff
  • Youth Church (2008) This includes Churches, Church Classes and Attendance, Events, Teachers and Youth
It also uses some ideas from work I did with a Local Authority in London on Youth Offenders.
